• Massage Therapy Establishment Exemption Memo

• Complete the application.
• Current professional and general liability insurance coverage for bodily injury and property damage for the establishment in the amount of at least $1,000,000 [Rule 532-X-3-.04(5)(a)]. The establishments name must be on the policy to ensure coverage. Some insurance organizations provide coverages for both an LMT and the Establishment. *Upload Certificate of Insurance.
• Application & License Fee $100 for two-year license. Process payment online.
• Background check required. $42 fee paid separate from license fee and can only be done through this online application. No third-party background reports are accepted. Please check your email for the background check information.

Each Establishment, whether onsite or an out-call establishment, shall obtain a license to operate from the Board [Rule 532-X-3-.04(1)].

The applicant for licensure of the establishment shall be the owner, lessee, or legal possessor of the establishment. [Rule 532-X-3-.04(2)]

Each massage therapy establishment applicant shall pass a criminal history check unless the applicant is a licensed massage therapist and has passed a criminal history check. [Rule 532-X-3-.04(9)].

An establishment that is owned by an individual, company, or corporation residing outside of the State of Alabama must be inspected prior to opening the establishment. [Rule 532-X-3-.04(11)]

• Current professional and general liability insurance coverage for bodily injury and property damage for the establishment in the amount of at least $1,000,000 [Rule 532-X-3-.04(5)(a)]. In the event the licensed establishment sold, subleased, or legal possession of the establishment is changed, the new owner, lessee, or legal possessor of the establishment shall be required to apply for and obtain a new license within sixty (60) days of the change of ownership, lessee, or legal possess.
