(LMT) Massage Therapist License - New

This online application process will require you to upload files. All documents need to be saved on your device as a file prior to starting the online process. Files to be upload is asterisk below (*).

1) Complete a minimum of 650 hours from an Accredited/Licensed Massage Therapy School. Out-of -State Schools are verified through NCBTMB or evidence of licensure by the state in which it is located. *Upload a copy of your transcript.
Generally, a maximum of 150 Continuing Education Hour may be used for transcripts under 650 hours. This must be approved by the Board’s Executive Director first.
2) Pass the MBLEx or NCBTMB Examinations. *Upload evidence of examination results.
3) Evidence of $1,000,000 Professional and General Liability Insurance Coverage [Rule 532-X-3-.03(5)]. *Upload Certificate of Insurance.
4) Background check required. $45 fee paid separate from license fee and can only be done through this online application. No third-party background reports are accepted.
5) Application & License Fee for two-year license $125. Process payment online.


(LMT) Massage Therapist License – Renewal

This online application process will require you to upload files. All documents need to be saved on your device as a file prior to starting the online process. Files to be upload is asterisk below (*).

1) Must have 16 CE Hours – certificates must be saved to upload. Do not attempt to upload certificates as separate files. *Upload one continues file of Certificates.
2) Evidence of $1,000,000 Professional and General Liability Insurance Coverage [Rule 532-X-3-.03(5)]. *Upload Certificate of Insurance.
3) Background check required. $45 fee paid separate from license fee and can only be done through this online application. No third-party background reports are accepted.
4) Application Fee for two-year license $100. Process payment online.


Massage Therapy Establishment License

This online application process will require you to upload files. All documents need to be saved on your device as a file prior to starting the online process. Files to be upload is asterisk below (*).

1) Complete the application.
2) Evidence of Liability Insurance for bodily injury and property damage (no specific amount) for an establishment [532-X-3-.04(5)(a)]. Some insurance organizations provide both coverages for an LMT and Establishment License. *Upload Certificate of Insurance.
3) Complete a background check $45 – if the owner is a licensed massage therapist, an additional background check is not necessary [532-X-3-.04(9)].
4) Application & License Fee $100 for two-year license.
5) Each Establishment, whether onsite or an out-call establishment, shall obtain a license to operate from the Board [532-X-3-.04(1)].
6) If Establishment moves to different location or Establishment is sold, a new application must be submitted, and new license number assigned. Such change shall be reported to the Board within 60 days. The current Establishment license cannot be transferred to another location nor transferred to new owner [532-X-3-04(3).

Establishment Owners do not have to be a licensed massage therapist BUT must complete a background check for the application to be complete. An LMT that has already obtained a background check does not have to obtain a second one for the Establishment License.
